| Patients' Rights As a Patient of Martha Jefferson Hospital, You Have Certain Rights. These Include the Following: - To be provided information from your physician about your condition, diagnosis and treatment so that you can make informed decisions.
- To be provided information on medically significant alternatives to treatment.
- To be provided with the names of persons responsible for your treatments and procedures.
- To be involved in developing his/her plan of care, including pain management.
- To be provided the opportunity to express your choices in advanced directives, such as a "living will" or naming a person to be your "proxy" who has the authority to make decisions regarding your health care.
- To be allowed to refuse treatment.
- To have your privacy safeguarded and your records kept confidential from those not involved in your care.
- To accept or refuse to take part in medical research as part of your care.
- To participate in decisions regarding transfer to another facility.
- To be provided education and information to care for yourself following discharge.
- To have access to your hospital bill, a full explanation of charges, and help in resolving any conflicts about your bill.
- To receive prompt and reasonable responces to requests for services.
- To be cared for in a safe environment.
- To have the ability to consult with a specialist at your request and expense, irrespective of your insurance plan.
- To have any conflicts dealt with fairly and reasonably and to have access to the Patient Advocate, the Senior Administrator on call, or the Ethics Consultation Service.
- To be free from restraints of any form that are not medically necessary.
